Auto-stamp or migrate on startup

On boot, run_migrations() checks the Alembic revision:
- no revision + tables exist  → stamp head (db.sql was run manually)
- no revision + no tables     → upgrade head (fresh install)
- revision present            → upgrade head (no-op if already current)
